What can I use as proof that I submitted my Form 1040?

If you will be using your Form 1040 as your proof of address document, you need to submit proof that it was submitted.

You must provide one of the following documents:

• Self-prepared
  • Email confirmation from online tax service like TurboTax or FreeTaxUSA
    • Must be a PDF. The bank will not accept a screenshot
• Prepared by an accountant
  • Form 9325
  • Completed bottom portion of the Form 1040
    • Must include the dates, customer signatures, and paid preparer’s name and signature
  • Accountant e-file Activity Report
    • Must include the submission IDs and dates

Document requirements

• First name and last name on the document must match the name on your passport.
• Tax return document must be from the previous year only.
